How do I get services?
heading_mp3People receive support after an assessment has been completed by a nurse or social worker in the Community Support Team.

During the assessment, they will discuss with you (and your carer/s) any difficulties you may have and what kind of support will be most helpful to you.

A set of rules called 'eligibility criteria' will be used to make sure that priority is given to people with the most urgent needs or who are at greater risk of coming to harm.
