Information about my support
When you live in your own home, you will get support to help with the things you need to do everyday.
There are two sorts of support you might have. These are:
- Support to help you live in your home and
- Support to help you live your life.
The Consortium does not have to give you both these types of support, you can choose for somebody else to provide you either of these types of support if you wish.
Let's see what these two sorts of support mean.
The first one is, Support to help you live in your home.
This means that the consortium will help you with things like:
- Understanding your tenancy agreement,
- Paying your rent and other bills
- Getting on with the other people you live with
- Keeping your home well looked after.
This support means that you will be able to live in your home for as long as you want.
The second type of support you might want the Consortium to provide is Support to help you live your life
This means getting help with things like:
- Learning new skills at home like shopping, cooking and cleaning
- Help to do things you enjoy, like making new friends.
- Any other help you need to do the things that make you happy.
This support will help you to live the life you want to.
When you receive support from the Consortium, you will meet regularly with Consortium staff, and your family, advocate, a social worker or community nurse, to decide what support you need.
You can say what support you would like, and you will all plan together what help and support you will have.
You can plan:
- What help you need to live in your home
- What help you want to live your life
- What sort of things you would like to do now (like swimming or going to the pictures)
- What sort of things you would like to do in the future.
The Consortium will write down the different sorts of support you agree to have to help plan your life. You will have different support plans for different parts of your life.
To help you remember what support you are getting from the Consortium, your Keyworker will keep all your plans, in your personal file.
You can ask your Keyworker to see your plans at any time, The plans will sometimes help you remember what support you have agreed to receive, and the decisions you have made.
To make sure you are getting the right sort of support from the Consortium, you can talk to your Keyworker or another staff member you get on with every month. This is called a Service Review.
Your Social Worker, advocate or close family member can also be involved if you want.
The service review will give you a chance to say how well your support service is going, and what you think about your support plans.
