What is Person Centred Planning?
Person Centred Planning is a way of working along side a person who wants to make changes in their life.
This involves bringing together all the people who are important to the person to listen and learn about them. A commitment is given to help them gain a more meaningful and fulfilling life.
Person Centred Planning is a way of finding out how a person wants to live, where they want their life to go and what needs to be done to get there.
Person Centred Planning assumes that all people have hopes and dreams and that their aspirations will change and grow with experience.
Who is Person Centred Planning for?
Anyone may use Person Centred Planning. There are no age limits. A person who has a specific need for support or anyone facing important life changes may find this process helpful.

‘All about me’ workbook
Some people have asked what the difference is between a personal passport and the ‘All about me’ workbook.
The personal passport is a tool e.g. a book, personal to an individual, with pictures and short snippets of information. It allows the individual to share the information that is important to him/her with others in his/her life.
It probably will not give you full details of how to communicate with an individual.
The ‘All about me’ workbook has much more detailed information.
It has been designed to make you think about things you like and things you dislike in your life.
