The Consortium
This building, at 24 Walter Road in Swansea, is the main office of the Community Lives Consortium.
The Consortium is a non profit making organisation, that provides care and support to people who have a learning disability and who wish to live in the Swansea, Neath and Port Talbot areas.
The Consortium will help people to live in ordinary houses, in ordinary streets.
If you get support from the Consortium, you will have the same rights as any other adult in our society.
Consortium staff will work together with your family members, or other people like your Social Workers or Community Nurses, to get the best possible advice and support for you.
The Consortium have people that meet to decide on how the Consortium is run. These people are called the Board of Management.
The Board of Management employs the Senior Managers. They are based in Walter Road and have responsibility for services provided by the Consortium.
The people who support you to live in your home are called Support Workers or Keyworkers.
You will have your own Keyworker to give you help and support.
Your Keyworker has a boss called the Service Coordinator. The Service Coordinator makes sure that you are happy with the support you get to live in your home.
The people who support you will always make sure you have a say in the support and service you receive from Consortium.
There are other ways that you can have a say to. Here are some of the ways you can have your say:
